PT-141 (bremelanotide, brand Vyleesi) is the only FDA-approved peptide for sexual health and the focus of this hub. It acts on melanocortin receptors in the central nervous system rather than peripheral vasculature, making it mechanistically different from PDE5 inhibitors like sildenafil and tadalafil. It is approved for hypoactive sexual desire disorder (HSDD) in premenopausal women and is widely used off-label by men.

Who this is for

Premenopausal women with HSDD per FDA labeling; men using PT-141 off-label for sexual health support, particularly when PDE5 inhibitors are contraindicated or ineffective. Best initiated under provider supervision with baseline blood pressure assessment.

Who should avoid

Anyone with uncontrolled hypertension or known cardiovascular disease — PT-141 produces transient blood pressure elevation. Pregnancy is a contraindication. Patients with severe nausea sensitivity may not tolerate the side-effect profile.

PT-141 acts centrally on melanocortin receptors in the brain — affecting desire and arousal pathways. PDE5 inhibitors (sildenafil, tadalafil) act peripherally on smooth muscle to support erectile mechanism. PT-141 can be effective when PDE5 inhibitors are not, and vice versa. They can be combined under supervision but the combination amplifies blood pressure changes.

Most sexual health pharmacology has gone the small-molecule route (PDE5 inhibitors, hormone therapy). PT-141 is the standout exception. Melanotan II is sometimes mentioned in this category but we do not provide a dosage chart for it due to documented melanoma case reports and regulatory warnings from the UK MHRA and Australian TGA.
